New Patients
To help make your first visit to our practice as smooth as possible, please read the information on this web page. You'll find all the practical information you need, such as practice hours, payment policies, and more. We also describe our first visit procedures. And, you can save some time by printing out and completing the new patient forms in advance of your appointment.
How to Become a Patient
Your insurance company may require that you get a referral from your Primary Care Physician before becoming a patient. Please check your policy. If required, you must get the proper authorization from your insurance company before scheduling your appointment.
Scheduling Your First Appointment
If a referral is required, the referring physician may schedule the appointment for you; or, once you receive the referral, you may schedule the appointment yourself by telephoning our office.
Preparing For Your First Appointment
When you come for your first appointment, please bring the following:
- Your insurance card,
- A list of your current medications,
- Your contact information (telephone number, mailing address, email address),
- Details of your medical history,
- Your co-pay.
